Documentation Required

  1. provincial/territorial health insurance card; 

  2. any one of the following proofs of identification;

  • Valid Canadian passport;

  • Birth certificate issued by a Canadian provincial, territorial or federal government agency;

  • Valid identification card, visa or permit with or without a picture, issued by a Canadianfederal, provincial, territorial government agency.
